Supporting Primary Care with Evidence-Based Digital Point-of-Care Tools
The Evidence2Practice Ontario (E2P) program is focused on digitizing quality standards in both primary care and acute care to support clinicians in delivering high-quality care across the province. E2P is co-led by the Centre for Effective Practice, the eHealth Centre of Excellence and North York General Hospital. E2P is funded by Ontario Health as part of the province’s Digital First for Health Strategy.
This presentation will provide a brief overview of the development and implementation of Ontario Health’s Quality Standards for heart failure, anxiety disorders, major depression, diabetes, and COPD into clinical decision support tools embedded into existing primary care workflows. The presentation will then dive into a demonstration of three E2P tools for primary care: COPD for Telus PS Suite, Anxiety Disorders and Major Depression for Accuro QHR, and Heart Failure for OSCAR Pro. Finally, a clinician end-user will join to provide their clinical perspective and experience using E2P tools and change management supports.
