SUMMARY:Webinar Wednesday - User-centric design as a key driver in bridging disconnected care
DESCRIPTION:Disconnected care remains a major challenge in healthcare\, limiting seamless communication and collaboration among providers\, patients\, and community resources. Bridging this gap requires innovative solutions rooted in interoperability and a user-centric approach to product design and development. By focusing on the needs of patients\, providers\, and administrators\, we can create solutions that are not only technologically advanced but also intuitive and impactful. \nCalian’s digital health experts will highlight the importance of user-centred design principles and their role in driving connected care\, underpinned by actionable takeaways for stakeholders in healthcare innovation. \nTakeaways \n\nGain insights into the user experience perspective and design considerations for technology tools aimed at closing gaps in care.\nUnderstand the critical importance of connected care for clinicians\, who rely on comprehensive patient information\, and for patients\, whose outcomes depend on it.\nExplore how partnerships with community programs can promote preventative\, proactive care\, moving beyond reactive approaches to drive long-term health improvements.\nDiscover how analyzing regional data—such as PHQ-9 or GAD-7 scores—can assess and enhance the impact of accessible care programs.\n\nJoin us explore how innovative solutions like Calian Corolar Virtual Care are transforming healthcare delivery\, creating a future where care is connected\, efficient\, and truly patient-centric.

SUMMARY:Webinar Wednesday - Nova Scotia and YourHealth NS - Empowering Citizens with their own health data
DESCRIPTION:Nova Scotia is leading the way in Canada to truly empower and enable citizens to access their health information anywhere\, any time through the YourHealthNS app. This represents a significant milestone in advancing connected data sources and systems through data interoperability standards. Nova Scotia is accelerating progress as per the Canada Health Infoway Pan-Canadian Interoperability Roadmap that was released in 2023. \nHear from Nova Scotia Health’s CIO\, Scott McKenna\, about how they are leveraging internal support and strong and positive collaboration with partners\, including innovative industry leaders\, to effect change. He will share key learnings and insights that could be helpful to other provinces as they continue their respective digital health and interoperability journeys.

SUMMARY:Webinar Wednesday - Optimizing patient referrals for cardiac imaging using AI
DESCRIPTION:Coronary artery disease (CAD) is the leading cause of death worldwide. For decades\, the gold-standard test used to diagnose CAD was invasive coronary angiography (ICA). However\, nearly half of the patients who receive ICA are found to have either no disease or non-significant CAD\, meaning that while they receive a definitive diagnosis\, they do not receive any therapeutic benefit. This is of concern because ICA is an expensive procedure\, and it carries a small but real risk to patients (roughly 1/1000 ICAs results in stroke\, heart attack and/or death). More recently\, a non-invasive diagnostic test\, cardiac computed tomographic angiography (CCTA)\, has been shown to be as effective as ICA at diagnosing CAD in the right patient population\, while being significantly less expensive and less risky for patients. \nOur team has used Artificial Intelligence (AI) to develop and validate a predictive model for determining which patients should proceed directly to ICA versus which should receive CCTA. We have also developed and tested a centralized triage pathway for ICA and CCTA to direct patients to the most appropriate test and validated its acceptability to both patients and referring physicians. We are now evaluating the AI tool combined with the centralized triage pathway through a multisite randomized control trial at Hamilton Health Sciences and Niagara Health. This model of care will reduce risk to patients\, reduce wait times for ICA and reduce costs to the health care system.
