SUMMARY:Commercializing an Artificial Intelligence-based Clinical Decision Support System for Patients with Coronary Artery Disease: Opportunities and Challenges
DESCRIPTION:Coronary artery disease is the leading cause of death globally. Although clinical practice guidelines on coronary revascularization exist\, clinicians still find it challenging to decide among medical therapy alone\, percutaneous coronary intervention\, and coronary artery bypass graft surgery due to unique patient characteristics and preferences. By leveraging rich\, large-scale patient data assets available in Alberta\, we have been developing and commercializing an artificial intelligence-based clinical decision support system to provide personalized\, data-driven treatment decision support at the point of care. This talk will discuss the opportunities and challenges that we have and will continue to face in our commercialization path\, as well as why commercialization is a good way to develop clinically impactful digital health solutions. \nThis webinar is presented in collaboration with Alberta Innovates and the AICE Concepts Program.

SUMMARY:Virtual Waiting Room at Urgent + Primary Care Centres: Improving Health Outcomes for Chemotherapy Patients Using Digital Remote Symptom Monitoring
DESCRIPTION:Part 1\nUrgent and Primary Care Centres (UPCC) across Vancouver Coastal Health were previously experiencing increased capacity and congestion in their waiting rooms. Overcrowded waiting rooms further increased patient’s frustration and anxiety when visiting the UPCC. Since the implementation of a virtual waiting room\, waiting room congestion has decreased and has significantly improved patient satisfaction and experience. \nA virtual waiting room was implemented across Urgent and Primary Care Centres (UPCC) in Vancouver\, Richmond and North Shore to manage their waiting room capacity by allowing clinically appropriate patients the flexibility to wait at an alternative location. Once the UPCC team is ready for the patient\, they alert the patient via a solution called Waitwhile\, which texts the patient instructing them to return to the UPCC. \nPart 2\nWhen Cancer patients undergoing chemotherapy can accurately and promptly report their symptoms and the care team is optimized to respond\, symptoms will be better managed leading to improved quality of life and decreased acute care visits and hospitalizations. Better management of side effects can also help to prevent treatment delays which will help to improve utilization of scarce chemotherapy chair spots. With this\, Richmond Hospital’s cancer team at Vancouver Coastal Health saw an opportunity to improve patient care by creating a remote symptom monitoring (RSM) system in conjunction with reorganization of the nursing team responsibilities by creating a new symptom management role \nThe hospital’s full-service outpatient cancer care clinic went live with a RSM monitoring system named RESPONSe in January 2024. Multiple design cycles included incorporation of conditional logic\, COSTARS oncology symptom specific questionnaires\, automatic triaging flags\, immediate self—management tips and links to educational resources. Over 30 patients have enrolled to date\, and early results show high completion rates of the health check and doctor’s visit surveys which have resulted in a response to 19 severe and 34 moderate reported side-effects and 63 symptom management phone calls by a dedicated symptom management nurse.

SUMMARY:The Technical and Ethical Complexities of Genomic EHR Integration
DESCRIPTION:Molecular genetics technology has rapidly advanced over the last decade with significant acceleration as a result of SARS-CoV2. Improved testing platforms\, lowered costs\, and a wider range of options available for patients have outpaced the ability of electronic health records to store and make use of genetic data. This session will discuss the complexities surrounding the life cycle of genomic information from the time of reporting to its incorporation into the EHR\, to longer-term storage and access control. \nLearning objectives include: \n\nIdentifying the technical challenges associated with accurately incorporating genetic results into the EHR for medical decision making;\nIdentifying the confounders to that integration;\nDescribing ethical “best practices” for genomic data use in an EHR (especially for vulnerable populations); and\nConsidering the potential legal implications of genomic data storage in the EHR and issues around access\, information sharing\, and non-discrimination.

SUMMARY:Secure Medical Records Sharing: Innovations in Federated Learning for Disease Surveillance
DESCRIPTION:Electronic Medical Records are secure\, private\, lifetime records containing patient-health and point-of-care histories within the healthcare system\, which can be used for disease surveillance\, clinical studies\, and many others. It is hard to share and use EMR data across jurisdictions because of the differences and barriers in data governance and architecture\, as well as data security regulations. Federated learning is a new machine learning paradigm that trains machine learning models collaboratively without centralizing the data or exchanging data among clients\, such as hospitals. Federated learning can broaden data used for analysis while honouring the required ethical and data sharing agreements. \nAlthough sepsis is a major worldwide health problem\, identifying sepsis remains challenging. The definitions for sepsis have been changing over the past 25 years. There is no gold standard definition for sepsis. Wide variation in incidence rates results from sepsis identification beginning with a pre-determined clinical definition and searching for cases in administrative and/or EMR data. Published work primarily utilize limited site-specific administrative data or registry data collected specifically for sepsis case-finding\, presenting significant limitations on method generalization. \nWe propose to develop a novel Federated Learning framework for disease surveillance\, with applications to the detection of sepsis\, using electronic medical record data across multiple provinces in Canada without data sharing. We will evaluate the performance of the proposed framework through comparing with manual chart review\, traditional locally trained machine learning models\, as well as the commonly used coded definition of sepsis. The developed Federated Learning framework based on electronic medical records can be used to routinely detect sepsis for disease surveillance and monitoring and facilitate decision-making. \nThis webinar is presented in collaboration with Alberta Innovates and the LevMax Health Program.
