SUMMARY:Webinar Wednesday: MotionInsight: An AI-Driven Functional Assessment Tool for Post-Stroke Care Personalization at Point-of-Need
DESCRIPTION:Comprehensive assessments of motor function are crucial for effective decision-making in neuromuscular rehabilitation. This is particularly critical post-stroke\, where personalized therapy drives an individual’s abilities to be ambulatory and return to their community in the short term and effectively use their upper limbs in the long term. Current clinical practice\, however\, relies on self-reporting\, subjective observation\, or basic scales\, which are limited by inconsistency\, low sensitivity\, ceiling effects\, and continuous reliance on clinical personnel. Also\, such assessments are typically conducted in larger clinical centres\, posing accessibility challenges for many living in rural areas or long-term care homes. \nWe propose MotionInsight\, a cost-effective\, accessible\, and user-friendly smartphone-based technology for objectively assessing motor function at point-of-need. Using artificial intelligence (AI)\, MotionInsight can objectively drive the personalized rehabilitation of gross/fine motor function of the upper arm/hand as well as balance in acute\, inpatient\, outpatient\, community-based\, and at-home settings. \nMotionInsight encompasses an app that guides users through personalized assessments or training tasks and computer vision techniques to quantitatively track body segments. It also includes a secure database and website for data management and progress tracking. Using the latter\, clinicians can prescribe new assessment and training tasks or kinematic metrics; and stroke survivors can track their progress. \nMotionInsight addresses an unmet clinical need by proposing an adoptable solution that only relies on an end-user smartphone. MotionInsight strengthens Canada’s digital health ecosystem by accelerating innovations in digitally-informed and evidence-based clinical decision-making for care personalization post-stroke.

The study evaluated the financial impacts of a Healthcare Orchestration Platform\, focusing on direct cost reductions associated with ER visits from a health system perspective. \nYou will gain an understanding of how a healthcare authority\, in collaboration with Petal\, a leading digital company\, successfully streamlined a series of processes aimed at reducing ED overcrowding. This session will highlight three strategic processes: 1) improving care access for unattached patients\, 2) ensuring timely appointments for urgent cases\, and 3) redirecting non-urgent visits away from EDs. These have collectively reduced over 800 ED visits per day – alleviating strain on hospitals\, improving patient flow\, and generating annual financial benefits of $73.8M. \nDiscover how a connected system that centralizes data from nearly 20 IT systems— including provincial authentication systems\, EMRs/EHRs\, and booking portals—can successfully optimize access to primary care and ensure patients receive appropriate care more efficiently. \nThis session will offer a unique opportunity for healthcare leaders to explore how strategic initiatives combined with innovative technology and intelligent processes can reshape care delivery and set new standards for sustainable\, patient-centred healthcare.

SUMMARY:Webinar Wednesday - Addressing Physician Shortages in Mid-Sized Rural and Remote Hospitals: The Virtual MRP Model
DESCRIPTION:Physician shortages pose a critical challenge in rural and remote hospitals across B.C.\, leading to diversions\, closures\, and operational challenges. Our presentation will provide an overview of an innovative and collaborative approach to addressing physician shortages in BC: Virtual MRP for Mid-Sized Hospitals. The Virtual MRP (Most Responsible Provider) for Mid-Sized Hospitals model integrates virtual physicians into in-person care teams utilizing remote technology to enable patient care including assessment\, diagnosis\, referrals\, and treatment. \nIn partnership\, Interior Health\, Northern Health\, Emergency Care BC\, the Ministry of Health\, and the Provincial Health Services Authority’s Provincial Virtual Health team have developed this model which aims to incorporate additional physician support into hospitals to stabilize hospital operations\, address physician shortages\, and reduce strain and burnout on in-person staff. This model will reduce service disruptions and enhance access to consistent\, timely care. The model is being piloted at Cariboo Memorial Hospital (CMH) in Williams Lake\, B.C.\, and scheduled to be piloted in Prince Rupert in early 2025; these pilots will inform scale and enable expansion to other sites across B.C. \nOur presentation will highlight key activities taken as part of the development of this model\, including: \n\nLearning from experts in other jurisdictional across Canada where similar models have been implemented with success.\nActively collaborating with key partners including hospital physicians\, nurses\, allied health\, clinical operations\, IMIT teams\, indigenous partners\, patient partners\, and hospital leadership.\nStrong application of the ADKAR model for change management and an innovation framework for all engagement\, sessions\, planning\, and implementation.\nIdentifying clinical\, technical\, and operational requirements.\nContracting and collaborating with a third-party vendor.\nDeveloping a readiness checklist and implementation playbook for new pilot sites.\nLeveraging learnings from the pilot sites to inform provincial scale and spread of the model.\n\nWe will provide an overview of the key features and requirements of the Virtual MRP for Mid-Sized Hospital model including options for: virtual provider staffing\, technology to facilitate virtual care\, a technology implementation and support process\, new clinical operational workflows\, a process for coordinating patient visits and for sharing clinical information including medical record\, and on-site staff to support the hands-on clinical interventions required at the site. We will discuss preliminary evaluation metrics from Cariboo Memorial Hospital and how lessons learned have been leveraged to other pilot sites and to help inform the provincial model of care. These learnings will be leveraged for continuous quality improvement. Findings will offer valuable and actionable insights to refine the model to ensure sustainability for long-term resilience against provider shortages and guide scaling and expansion of the model hospitals across B.C.

SUMMARY:Webinar Wednesday - Optimizing patient referrals for cardiac imaging using AI
DESCRIPTION:Coronary artery disease (CAD) is the leading cause of death worldwide. For decades\, the gold-standard test used to diagnose CAD was invasive coronary angiography (ICA). However\, nearly half of the patients who receive ICA are found to have either no disease or non-significant CAD\, meaning that while they receive a definitive diagnosis\, they do not receive any therapeutic benefit. This is of concern because ICA is an expensive procedure\, and it carries a small but real risk to patients (roughly 1/1000 ICAs results in stroke\, heart attack and/or death). More recently\, a non-invasive diagnostic test\, cardiac computed tomographic angiography (CCTA)\, has been shown to be as effective as ICA at diagnosing CAD in the right patient population\, while being significantly less expensive and less risky for patients. \nOur team has used Artificial Intelligence (AI) to develop and validate a predictive model for determining which patients should proceed directly to ICA versus which should receive CCTA. We have also developed and tested a centralized triage pathway for ICA and CCTA to direct patients to the most appropriate test and validated its acceptability to both patients and referring physicians. We are now evaluating the AI tool combined with the centralized triage pathway through a multisite randomized control trial at Hamilton Health Sciences and Niagara Health. This model of care will reduce risk to patients\, reduce wait times for ICA and reduce costs to the health care system.

SUMMARY:Webinar Wednesday - Advancing Paramedic Training and Assessment through Virtual Reality: Saskatchewan's College of Paramedics VAPOC Project
DESCRIPTION:There is a paramedic shortage in Saskatchewan. The Government of Saskatchewan has allocated $10.8 million in the 2022-23 budget to fill more Emergency Medical Services positions\, mainly in rural and remote areas. However\, barriers to licensure limit workforce supply. Paramedic assessments are limited to on-site evaluation\, posing undue burden and financial risk to applicants\, especially foreign-trained ones. \nCommunity paramedics\, who deliver short-term care for low-acuity illnesses\, alleviate pressure on ER wait times and hospital beds but constitute less than 1% of all licensed paramedics in Saskatchewan. The urgency due to COVID-19 has opened a policy window for innovation and regulatory change in virtual technology for education and licensure. One promising innovation is virtual reality (VR) simulation. \nThe Saskatchewan College of Paramedics has developed high-fidelity VR simulations that assess 70% of the competencies required for entry to practice. However\, VR use in regulatory contexts is a new concept that warrants further exploration. Our research team has been evaluating its usability and potential applications for regulatory purposes. \nThis webinar aims to present preliminary findings from two innovative projects leveraging VR technology to enhance paramedic training and assessment in Saskatchewan. We will explore future directions involving engagement with regulators on recognizing VR as an acceptable practice.

DESCRIPTION:Chronic obstructive pulmonary disease (COPD) affects more than 2.5 million Canadians\, half of whom will not be properly diagnosed until they reach an advanced stage of the disease. Clinicians need easier point-of-care access to evidence-based information to support faster\, appropriate diagnoses and improve patient health outcomes. \nThe Evidence2Practice Ontario (E2P) program offers a free EMR-integrated COPD tool for TELUS PS Suite and Accuro QHR (with an OSCAR Pro version to be released this fall) that supports easier access to the information at the point of care. \nIn a demonstration of the E2P tool for COPD’s key features\, you’ll learn more about: \n\nCOPD diagnosis and how the tool supports the confirmation of diagnosis by spirometry and captures results.\nDisease management support and the embedded accessory instruments to measure the degree of disease burden. You’ll also see how the tool tracks acute exacerbations\, graphs timing\, reviews\, and captures severity to determine the risk of future acute exacerbations.\nMedication plans that provide decision support related to recommended pharmacologic treatments.\n\nA primary care clinician and E2P user* will join us to share their experience using the tool with patients\, highlighting their favourite features and optimization tips. \nE2P implementation support staff will also be on hand to answer your practical and clinical questions.

SUMMARY:Qualitative Exploration of Emergency Department Nurse and Physician Experiences with the Ontario eHub HIE
DESCRIPTION:Electronic Health Records (EHRs) have improved information access and care continuity. Despite the benefits of EHRs\, Health Canada acknowledges the difficulties healthcare clinicians face in sharing or accessing information due to a lack of EHR interoperability. The lack of interoperability across settings can lead to unnecessary duplication of diagnostic testing\, longer wait times\, and extended hospital stays. \nIntegrating Health Information Exchange (HIE) into EHRs improves interoperability as it allows seamless information sharing between healthcare providers and settings\, creating a comprehensive picture of a patient’s health history through the healthcare system. The Ontario eHub HIE was launched to ensure seamless information transfer among healthcare providers by compiling patient data in a repository that can be queried into existing EHRs. Emergency departments rely heavily on access to comprehensive data; therefore\, it is important to evaluate the nursing and physician experiences of HIE. \nThis study aims to evaluate the Ontario eHub’s usability\, efficiency\, and impact on workflow for LHSC emergency department (ED) nurses and physicians approximately one year after implementation. \nThis study will adopt the interpretive description (ID) methodology and employ purposive sampling to recruit ED nurses and physicians. Grounded in the UTAUT model\, the study examines how the various concepts within UTAUT influence HIE utilization in ED. Data collection will be through one-on-one audio-recorded semi-structured interviews. Inductive thematic analysis will be utilized to analyze the participant data\, which aligns with ID methodology. \nFindings from this study can highlight how the Ontario eHUB HIE integrates into the workflows of ED nurses and physicians\, identifying areas for optimization. It can also inform future HIE expansions to other healthcare settings\, as it is currently limited to long-term care and acute care centres.

SUMMARY:Essential Digital Health for the Underserved: Establishing an Equitable Learning Health System for Virtual Care
DESCRIPTION:The Learning health system (LHS) approach is a popular framework that many policy makers and health administrators are employing to introduce continuous quality improvement and improve the cost effectiveness of health services in their jurisdictions. How can this LHS approach engage communities\, patients and industries to join health policy makers and health professionals to increase equity of access of care? \nThis presentation will introduce how a national technology enabled care conference turned its attention to using digital health to support equity deserving populations\, and how to introduce the deliberative dialogue method to focus on equity-informed virtual care in BC. A dialogue was held last year with Indigenous and rural community members\, policy makers\, health administrators\, health practitioners\, and health services researchers. The central aim was to advance health equity for rural\, remote\, and Indigenous communities\, with the dialogue centered on optimizing patient journeys and community experiences so as to develop a set of recommendations for all relevant stakeholders to collaborate on the delivery and improvement of virtual care. This deliberative dialogue approach is essential in the relationship and trust building needed yet often overlooked in LHS framework. \nThe presenters will show how UBC Digital Emergency Medicine incorporates deliberative dialogue into our LHS tool kit\, and invite participants to share their experiences to promote knowledge exchange.

SUMMARY:Webinar Wednesday - SmartParent: Teaching by Texting
DESCRIPTION:In Canada today\, less than 30% of expectant parents attend childbirth classes. Barriers to attendance include cost\, lack of instructors\, inclement weather and geography in rural areas\, and stigma. formation online is overwhelming and contradictory. \nPeople are looking for trustworthy information. Attendance at prenatal education classes has been associated with positive health behaviors\, including appropriate weight gain\, attendance at prenatal care\, acceptance of routine screening tests\, smoking cessation\, decreased alcohol consumption\, and breastfeeding. Failure to achieve these behaviors in turn has been associated with adverse maternal and newborn outcomes including cesarean birth\, preterm birth\, and inadequate fetal growth. \nThis webinar will present SmartParent\, Canada’s first prenatal and parenting education program delivered through text messaging. \nSmartParent makes evidence-based\, relevant and timely information available to all parents\, regardless of location\, socioeconomic status\, and education. Text messages containing comprehensive information relevant to each week of pregnancy or baby’s age are delivered to mobile devices three times weekly. Each message contains links to additional online resources. SmartParent users can opt in to supplementary information streams to tailor the program to their own needs. \nUnlike other digital solutions such as apps\, blogs\, and websites\, our text messages bring the right message to the right person at the right time. SmartParent users have lower rates of depression and fear of childbirth\, less use of cannabis and tobacco\, and reduced rates of complications including diabetes and inadequate fetal growth\, and improved confidence as new parents.
