SUMMARY:Webinar Wednesday - AI Scribes: Findings from Ontario's Clinical Evaluation Study
DESCRIPTION:In this webinar\, attendees will learn from panelists about the findings and lessons learned of the recently-published AI scribe clinical evaluation report produced by Women’s College Hospital Institute for Health System Solutions and Virtual Care (WIHV) on behalf of OntarioMD. OntarioMD led the initiative with 150 primary care clinicians over a three-month period to study how they use different AI scribes and what benefits they saw during the study. Clinicians used AI scribe tools to transcribe their conversations during patient encounters into accurate and complete SOAP notes. The clinicians saved time each week on administrative tasks. They also spent at least much less time on paperwork during clinic hours and after hours (“pyjama time”) and they dramatically reduced their cognitive load during patient encounters. Clinicians could maintain face-to-face contact with patients\, never looking at their computer screens until it was time to check the SOAP note for accuracy. Clinicians reported in addition to all the benefits of AI scribes that this important outcome is increasing the quality of patient visits and restoring their joy in medicine. \n

DESCRIPTION:Webinar Wednesday – Impact of Teleconsultation Implementation in Small Rural Long-Term Care Facilities: A pilot study on Nurses’ and Nursing Assistants’ Experiences \nIn Quebec\, the shortage of nurses during night shifts in small rural long-term care facilities (LTCFs) poses a serious risk to resident safety and care quality. This issue is exacerbated by the province’s broader nursing shortage\, making it difficult to maintain adequate staffing. To address this challenge\, some facilities have implemented on-call telephone services to ensure 24-hour nursing care access. In response\, Quebec’s Direction nationale des soins et des services infirmiers (DNSSI) launched a teleconsultation pilot project. \nThis study explored the experiences of nurses and nursing assistants integrating teleconsultation in rural LTCFs with fewer than 50 residents. The six-month project was implemented in three facilities across two administrative regions between July 2022 and March 2023. A total of 18 semi-structured interviews were conducted with nine participants from February to July 2023.\nFindings revealed that teleconsultation significantly improved clinical\, administrative\, and organizational practices while maintaining stability in certain workflows. It optimized processes by expanding nursing assistants’ scope of practice\, ensuring safer and more efficient care. Collaboration between nurses and nursing assistants was enhanced\, leading to better care coordination and communication. Leadership played a crucial role in defining professional roles and supporting the transition. \nThis initiative represents a major advancement in improving LTCF care in Quebec. Given its success in addressing staffing shortages and enhancing care quality\, the pilot project might be expanded to additional regions to refine findings and further strengthen resident safety and care standards.

The disconnected nature of health information contributes to growing clinician stress levels and administrative burden\, and is a significant risk to patient safety. \nWorking together\, the CMA\, CFPC\, RCPSC\, and Canada Health Infoway convened a task force of clinical leaders from across Canada to identify barriers and solutions to physician involvement in interoperable digital health activities. \nThe task force produced a report that outlined key challenges and practical solutions in five policy recommendations\, required to advance connected care (interoperability). \nThis session will: \n\nBriefly review the recommendations of the Task Force.\nProfile new and emerging national and provincial/territorial activities intended to act upon the Task Force recommendations\, including the national AI Scribe Investment Program.\nEngage participants on the path forward\, by asking: how can clinicians be best supported to participate in a digitally connected healthcare system?

DESCRIPTION:Ten years later: The impacts of GetCheckedOnline\, BC’s digital testing program for sexually transmitted and blood borne infections\nWhen it launched in 2014\, GetCheckedOnline was the first publicly funded digital testing service of its kind in Canada. Operated by the BC Centre for Disease Control (BCCDC) and the BC Public Health Laboratory (BCPHL)\, GetCheckedOnline started in Vancouver and is now available in nine communities across the province\, serving over 22\,000 people per year. In its decade of operation\, the GetCheckedOnline program has been recognized as a Leading Practice by Accreditation Canada\, a runner-up in the BC Health Quality Awards and a Top Elite Winner by UNIVANTS of Healthcare Excellence. \nGetCheckedOnline’s objectives were to increase testing for sexually transmitted and blood borne infections (STBBI)\, diagnose infections earlier\, reach populations with higher rates of infections facing barriers to getting tested\, and improve the health system’s ability to provide STBBI testing in BC. Over the past 10 years\, GetCheckedOnline has had a significant impact on STBBI testing in the province and has successfully met its original objectives. In the current context\, driven by the ongoing digital transformation of healthcare\, the program must now explore how to remain sustainable while better meeting the needs of equity-deserving populations. \nIn this presentation\, we’ll discuss how GetCheckedOnline works and what we have learned about the population\, public health and health system impacts of this digital health service. We’ll share lessons learned from developing a digital sexual health program\, what we think is the key to GetCheckedOnline’s success\, and why flexibility and adaptability are essential to the future of this program.

SUMMARY:AI and Health Equity: Re-Imagining the Future of Clinical Decision Support
DESCRIPTION:Artificial intelligence (AI) is increasingly being explored to support a wide range of healthcare applications. Over the past decade\, a major focus has been on training machine learning models to predict clinical outcomes and risks\, with the goal of enhancing decision-making around assessment and care\, referred to as clinical decision support. The hope is that integrating predictions into the clinic at the point of decision-making will meaningfully impact patient care\, translating to more accurate assessment\, personalized treatment\, and targeted prevention or intervention. \nYet\, a growing body of evidence shows that AI models used in healthcare can reinforce or even amplify existing biases\, particularly against marginalized or underrepresented patient groups. Although clinical oversight\, or the presence of a “human in the loop”\, is often proposed as a safeguard\, it remains unclear whether this approach is sufficient to prevent AI from exacerbating health inequities when deployed. \nIn this presentation\, we explore the health equity implications of AI-based clinical decision support for assessing inpatient violence risk in acute psychiatric settings. Drawing on our interdisciplinary research at the Centre for Addiction and Mental Health (CAMH)\, we present findings from two studies that demonstrate how social and systemic biases can become embedded in training datasets\, which\, in turn\, influence AI model predictions in ways that disadvantage socially and racially marginalized patients. We also share insights from our experiments in human-computer interaction\, which suggest that clinical oversight may not be sufficient to mitigate these biases. We conclude with an overview of our future research\, which argues for the need to re-imagine how we design and deploy AI in contexts where it runs the risk of exacerbating existing health inequities.
