SUMMARY:Webinar Wednesday - Optimizing patient referrals for cardiac imaging using AI
DESCRIPTION:Coronary artery disease (CAD) is the leading cause of death worldwide. For decades\, the gold-standard test used to diagnose CAD was invasive coronary angiography (ICA). However\, nearly half of the patients who receive ICA are found to have either no disease or non-significant CAD\, meaning that while they receive a definitive diagnosis\, they do not receive any therapeutic benefit. This is of concern because ICA is an expensive procedure\, and it carries a small but real risk to patients (roughly 1/1000 ICAs results in stroke\, heart attack and/or death). More recently\, a non-invasive diagnostic test\, cardiac computed tomographic angiography (CCTA)\, has been shown to be as effective as ICA at diagnosing CAD in the right patient population\, while being significantly less expensive and less risky for patients. \nOur team has used Artificial Intelligence (AI) to develop and validate a predictive model for determining which patients should proceed directly to ICA versus which should receive CCTA. We have also developed and tested a centralized triage pathway for ICA and CCTA to direct patients to the most appropriate test and validated its acceptability to both patients and referring physicians. We are now evaluating the AI tool combined with the centralized triage pathway through a multisite randomized control trial at Hamilton Health Sciences and Niagara Health. This model of care will reduce risk to patients\, reduce wait times for ICA and reduce costs to the health care system.

SUMMARY:Webinar Wednesday - Addressing Physician Shortages in Mid-Sized Rural and Remote Hospitals: The Virtual MRP Model
DESCRIPTION:Physician shortages pose a critical challenge in rural and remote hospitals across B.C.\, leading to diversions\, closures\, and operational challenges. Our presentation will provide an overview of an innovative and collaborative approach to addressing physician shortages in BC: Virtual MRP for Mid-Sized Hospitals. The Virtual MRP (Most Responsible Provider) for Mid-Sized Hospitals model integrates virtual physicians into in-person care teams utilizing remote technology to enable patient care including assessment\, diagnosis\, referrals\, and treatment. \nIn partnership\, Interior Health\, Northern Health\, Emergency Care BC\, the Ministry of Health\, and the Provincial Health Services Authority’s Provincial Virtual Health team have developed this model which aims to incorporate additional physician support into hospitals to stabilize hospital operations\, address physician shortages\, and reduce strain and burnout on in-person staff. This model will reduce service disruptions and enhance access to consistent\, timely care. The model is being piloted at Cariboo Memorial Hospital (CMH) in Williams Lake\, B.C.\, and scheduled to be piloted in Prince Rupert in early 2025; these pilots will inform scale and enable expansion to other sites across B.C. \nOur presentation will highlight key activities taken as part of the development of this model\, including: \n\nLearning from experts in other jurisdictional across Canada where similar models have been implemented with success.\nActively collaborating with key partners including hospital physicians\, nurses\, allied health\, clinical operations\, IMIT teams\, indigenous partners\, patient partners\, and hospital leadership.\nStrong application of the ADKAR model for change management and an innovation framework for all engagement\, sessions\, planning\, and implementation.\nIdentifying clinical\, technical\, and operational requirements.\nContracting and collaborating with a third-party vendor.\nDeveloping a readiness checklist and implementation playbook for new pilot sites.\nLeveraging learnings from the pilot sites to inform provincial scale and spread of the model.\n\nWe will provide an overview of the key features and requirements of the Virtual MRP for Mid-Sized Hospital model including options for: virtual provider staffing\, technology to facilitate virtual care\, a technology implementation and support process\, new clinical operational workflows\, a process for coordinating patient visits and for sharing clinical information including medical record\, and on-site staff to support the hands-on clinical interventions required at the site. We will discuss preliminary evaluation metrics from Cariboo Memorial Hospital and how lessons learned have been leveraged to other pilot sites and to help inform the provincial model of care. These learnings will be leveraged for continuous quality improvement. Findings will offer valuable and actionable insights to refine the model to ensure sustainability for long-term resilience against provider shortages and guide scaling and expansion of the model hospitals across B.C.

SUMMARY:Industry Showcase Webinar: $73.8M in Annual Financial Benefits by Redirecting ED Visits - Results of an Economic Study
DESCRIPTION:Are your healthcare facilities grappling with the challenges of overcrowded Emergency Departments (EDs)\, overburdened care providers\, and limited patient access to timely care? These issues impact all stakeholders: patients struggle to access appropriate care\, hospitals face heightened operational demands\, and healthcare systems incur substantial costs due to non-urgent ED visits. \nThis session will showcase the results of a study that was conducted by Analysis Group\, one of the largest international economic consulting firms. The study evaluated the financial impacts of a Healthcare Orchestration Platform\, focusing on direct cost reductions associated with ER visits from a health system perspective. \nYou will gain an understanding of how a healthcare authority\, in collaboration with Petal\, a leading digital company\, successfully streamlined a series of processes aimed at reducing ED overcrowding. This session will highlight three strategic processes: 1) improving care access for unattached patients\, 2) ensuring timely appointments for urgent cases\, and 3) redirecting non-urgent visits away from EDs. These have collectively reduced over 800 ED visits per day – alleviating strain on hospitals\, improving patient flow\, and generating annual financial benefits of $73.8M. \nDiscover how a connected system that centralizes data from nearly 20 IT systems— including provincial authentication systems\, EMRs/EHRs\, and booking portals—can successfully optimize access to primary care and ensure patients receive appropriate care more efficiently. \nThis session will offer a unique opportunity for healthcare leaders to explore how strategic initiatives combined with innovative technology and intelligent processes can reshape care delivery and set new standards for sustainable\, patient-centred healthcare.

SUMMARY:Webinar Wednesday: MotionInsight: An AI-Driven Functional Assessment Tool for Post-Stroke Care Personalization at Point-of-Need
DESCRIPTION:Comprehensive assessments of motor function are crucial for effective decision-making in neuromuscular rehabilitation. This is particularly critical post-stroke\, where personalized therapy drives an individual’s abilities to be ambulatory and return to their community in the short term and effectively use their upper limbs in the long term. Current clinical practice\, however\, relies on self-reporting\, subjective observation\, or basic scales\, which are limited by inconsistency\, low sensitivity\, ceiling effects\, and continuous reliance on clinical personnel. Also\, such assessments are typically conducted in larger clinical centres\, posing accessibility challenges for many living in rural areas or long-term care homes. \nWe propose MotionInsight\, a cost-effective\, accessible\, and user-friendly smartphone-based technology for objectively assessing motor function at point-of-need. Using artificial intelligence (AI)\, MotionInsight can objectively drive the personalized rehabilitation of gross/fine motor function of the upper arm/hand as well as balance in acute\, inpatient\, outpatient\, community-based\, and at-home settings. \nMotionInsight encompasses an app that guides users through personalized assessments or training tasks and computer vision techniques to quantitatively track body segments. It also includes a secure database and website for data management and progress tracking. Using the latter\, clinicians can prescribe new assessment and training tasks or kinematic metrics; and stroke survivors can track their progress. \nMotionInsight addresses an unmet clinical need by proposing an adoptable solution that only relies on an end-user smartphone. MotionInsight strengthens Canada’s digital health ecosystem by accelerating innovations in digitally-informed and evidence-based clinical decision-making for care personalization post-stroke.

DESCRIPTION:Webinar Wednesday – Real-world effectiveness of self-led internet-based cognitive behavioural therapy (iCBT) versus standard CBT: a cohort study using Ontario’s linked health-administrative data \nBackground \nGrowing demands for mental health services have resulted in increased interest in virtual interventions delivered online and via mobile applications. Internet-based Cognitive Behavioural Therapy (iCBT) is an evidence-based intervention where users progress on their own time through online modules to modify maladaptive thinking and behaviour patterns and develop coping skills. While the efficacy of iCBT for anxiety and depression has been demonstrated in randomized controlled trials\, we leveraged linked\, health-administrative data to examine its real-world effectiveness\, compared to standard CBT. \nMethods \nIn this retrospective cohort study\, we compared individuals receiving iCBT (self-led\, online modules with minimal\, asynchronous therapist support) via a pilot program to those receiving standard CBT (scheduled\, one-one-one\, therapist-led sessions) in Ontario\, Canada between Jan 2020 and Aug 2021. We utilized a Difference-in-Differences analysis to assess pre-post change in depression (PHQ-9) and anxiety (GAD-7) symptoms and examined mental health-related service utilization (emergency department visits\, hospitalizations\, and outpatient visits to psychiatrists and primary care providers) using Logistic and Negative Binomial regressions for binary and count outcomes\, respectively. \nResults \nAmong N=167 individuals enrolled in iCBT and N=300 controls receiving standard CBT\, we found that treatment modality was a significant predictor of pre-post symptom change (p<0.01 for DID coefficients)\, with standard CBT clients experiencing a greater reduction in depression and anxiety symptoms (a 4-point reduction in PHQ-9 and 3.6-point reduction in GAD-7)\, compared to iCBT clients (2.3-point reduction in PHQ-9 and 0.35-point increase in GAD-7)\, though sensitivity analyses restricting to treatment completers somewhat attenuated this finding. While both groups experienced a reduction in MH-related service use\, iCBT clients were more likely to continue visiting both acute care (ORadj=2.92\, 95% CI 1.29-6.64) and outpatient physicians (RRadj=1.32\, 95% CI 1.03-1.69) 1-year post-treatment\, adjusted for age\, comorbidities\, and baseline symptom severity. \nConclusions \nIn routine care\, self-led iCBT appears to be less effective than standard CBT in terms of anxiety and depression symptoms and in terms of mental health-related service utilization. While iCBT remains a promising approach for leveraging technological innovation to meet access needs\, the benefits of regular therapist contact cannot be overlooked.

DESCRIPTION:We present a novel Neural-Symbolic Explainable-AI (XAI) framework\, integrating state-of-the art semantic knowledge graphs and machine learning (ML) methods\, to make the output of ML models transparent so they can be safely used in clinical settings. Current XAI methods explain the output of ML models simply in terms of feature importance. Our XAI framework generates health knowledge-driven decision paths\, comprising clinical concepts and evidence drawn from the literature\, to provide a clinically-interpretable trace of how the ML model has processed the input data to generate its output/predictions. We applied our XAI framework for organ transplantation decision support\, where we generate and visualize decision paths comprising organ transplant concepts to explain the ML-model’s predictions of kidney transplant survival and donor-recipient matches\, thus making complex ML models useful for organ transplantation.

SUMMARY:Webinar Wednesday - AI Scribes: Findings from Ontario's Clinical Evaluation Study
DESCRIPTION:In this webinar\, attendees will learn from panelists about the findings and lessons learned of the recently-published AI scribe clinical evaluation report produced by Women’s College Hospital Institute for Health System Solutions and Virtual Care (WIHV) on behalf of OntarioMD. OntarioMD led the initiative with 150 primary care clinicians over a three-month period to study how they use different AI scribes and what benefits they saw during the study. Clinicians used AI scribe tools to transcribe their conversations during patient encounters into accurate and complete SOAP notes. The clinicians saved time each week on administrative tasks. They also spent at least much less time on paperwork during clinic hours and after hours (“pyjama time”) and they dramatically reduced their cognitive load during patient encounters. Clinicians could maintain face-to-face contact with patients\, never looking at their computer screens until it was time to check the SOAP note for accuracy. Clinicians reported in addition to all the benefits of AI scribes that this important outcome is increasing the quality of patient visits and restoring their joy in medicine. \n

DESCRIPTION:Webinar Wednesday – Impact of Teleconsultation Implementation in Small Rural Long-Term Care Facilities: A pilot study on Nurses’ and Nursing Assistants’ Experiences \nIn Quebec\, the shortage of nurses during night shifts in small rural long-term care facilities (LTCFs) poses a serious risk to resident safety and care quality. This issue is exacerbated by the province’s broader nursing shortage\, making it difficult to maintain adequate staffing. To address this challenge\, some facilities have implemented on-call telephone services to ensure 24-hour nursing care access. In response\, Quebec’s Direction nationale des soins et des services infirmiers (DNSSI) launched a teleconsultation pilot project. \nThis study explored the experiences of nurses and nursing assistants integrating teleconsultation in rural LTCFs with fewer than 50 residents. The six-month project was implemented in three facilities across two administrative regions between July 2022 and March 2023. A total of 18 semi-structured interviews were conducted with nine participants from February to July 2023.\nFindings revealed that teleconsultation significantly improved clinical\, administrative\, and organizational practices while maintaining stability in certain workflows. It optimized processes by expanding nursing assistants’ scope of practice\, ensuring safer and more efficient care. Collaboration between nurses and nursing assistants was enhanced\, leading to better care coordination and communication. Leadership played a crucial role in defining professional roles and supporting the transition. \nThis initiative represents a major advancement in improving LTCF care in Quebec. Given its success in addressing staffing shortages and enhancing care quality\, the pilot project might be expanded to additional regions to refine findings and further strengthen resident safety and care standards.
