The Alberta Cancer Prevention Legacy Fund, in partnership with the Alberta Health Services’ Screening Programs and Information Technology Department, developed “Surfacing Population, Public and Indigenous Health (PPIH) Data for Action”, a three-year project with the goal of  finding an electronic solution to deliver patient information in a simple, easy-to-use format, that would identify the right action to take, in a system that was widely available across the province to clinicians regardless of the clinic or organization to which they belonged. This Case Study describes the solution they found.
